Does creatine help brain health and cognition?
Creatine may provide a small benefit for memory, particularly in some older adults, but broader cognitive benefits are unconfirmed. Meta-analyses of randomized trials found small pooled improvements in memory, while overall cognitive function and executive function did not improve significantly.[1], [2]
What the evidence shows
The evidence includes a limited number of small, heterogeneous trials. Studied amounts ranged from about 2.2 to 20 g per day over several days to several months, but the pooled analyses do not identify a reliable personal regimen.[1], [2]
Pooled trials found a small memory effect
Two meta-analyses of randomized controlled trials reported small improvements in memory with creatine compared with placebo. The standardized mean differences were approximately 0.29 and 0.31.[1], [2]
One review rated the certainty for memory as moderate. The effect was statistically significant but modest, and trial results varied substantially.[1], [2]
The effect appears stronger in older adults
In one subgroup analysis, adults aged 66 to 76 showed a larger and statistically significant memory benefit, while younger participants aged 11 to 31 did not show a significant change.[1], [2]
A second analysis also suggested that some clinical and middle-aged subgroups may benefit more, but these subgroup findings were based on relatively few studies and should not be used to predict an individual response.[1], [2]
Memory is not the same as cognition overall
Pooled analyses did not find a significant effect on overall cognitive function or executive function. Signals for attention and information-processing speed were rated low certainty.[1], [2]
The current evidence is therefore better described as a possible limited memory effect than as a broad cognitive-performance enhancement.[1], [2]
The studied protocols varied widely
Across the included trials, amounts ranged from roughly 2.2 to 20 g per day and intervention lengths ranged from several days to several months.[1], [2]
That range reflects heterogeneous experiments, not proof that all amounts are equivalent or that a high amount is more effective. The meta-analyses do not establish a best dose or timing strategy for cognition.[1], [2]
The evidence base remains small

A memory signal is not treatment of cognitive disease
The meta-analyses mainly summarize tests of cognitive performance. They do not establish prevention or treatment of dementia, depression, traumatic brain injury, or another neurologic condition.[1], [2]
People with cognitive symptoms or diagnosed neurologic disease need condition-specific medical assessment rather than extrapolation from small healthy-adult trials.[1], [2]
General safety conclusions have boundaries
Creatine kidney-safety findings are reassuring in generally healthy adults at studied amounts and durations, but do not cover pre-existing kidney disease. Higher cognition-study amounts should not be copied into a personal protocol without context.[1], [2], [3], [4]
The evidence also does not establish safety in pregnancy or breastfeeding for alternative forms.[5]
Important limitations
The pooled memory effect is small, heterogeneous, and based on a limited trial set. Broader cognition and executive function were not significantly improved, subgroup results are uncertain, and the studied amount range does not establish a personal cognitive protocol.[1], [2]
